While no surgery was required, he did have to go to the emergency room, get x‑rays, have the bone set, and then get fit for a cast to help it heal. If John had a Platinum plan, his monthly costs for the insurance were probably pretty high, but he would only have to pay $250 out of his pocket (10% of the $2,500) to get his arm back to normal.
If John had a Bronze plan, his monthly costs for the insurance would be much lower than the Platinum plan, but he would have to pay $1,000 out of his pocket (40% of the $2,500) to get that arm working again.
